Naranjo et al developed a score to help determine the probability that reaction in a patient is due to an adverse drug effect. This is based simple responses to 10 questions about the reaction and its relation to drug exposure.
The instrument consist of 10 questions.
Responses are: yes, no or don't know.
Points assigned for responses range from -1 to 2.

Interpretation:
• minimum score: -4
• maximum score: 13
• The higher the score, the more likely the drug is the cause for the adverse effect seen in the patient.

Performance:
• The kappa score for interobserver: 0.69-0.86; for intraobserver: 0.64-0.95.
Limitations:
• A drug causing an idiosyncratic reaction that was not dose-related might be hard to confirm by this approach, especially if the reaction was severe enough to preclude readministration.
