Description

Crohn's Disease needs to be distinguished from intestinal tuberculosis, especially in developing countries. Makharia et al developed a simple equation to aid in differentiating these 2 conditions. The authors are from All India Institute of Medical Sciences in New Delhi.

Parameters:

(1) involvement of the sigmoid colon

(2) blood in the stool

(3) weight loss (amount of loss not specified)

(4) focally enhanced colitis

where:

• Weight loss is associated with intestinal tuberculosis, while the remaining parameters are not.

score =

= SUM(points for the 4 parameters) + 7

Interpretation:

• minimum score: 0.3 (the addition of 7 is to avoid a negative number)

• maximum score: 9.3

• A score >= 5.1 indicates intestinal tuberculosis. To obtain this score there are 5 patterns: (1) weight loss only; (2) weight loss and sigmoid colon involvement; (3) weight loss and blood in stool, (4) weight loss and focally enhanced colitis, (5) weight loss and both blood in stool and focal colitis.

Performance:

• The area under the ROC curve is 0.91.
