Description

Poon et al reported risk factors for invasive Candidiasis in a patient who has undergone an intra-abdominal surgical procedure. The authors are from New York Presbyterian Hospital-Columbia in New York City.


Patient selection: status post intra-abdominal surgery, currently in the intensive care unit

 

Most common intra-abdominal procedures: bowel resection, bowel suture, open surgery with lavage

 

Candida species most often isolated: Candida albicans and Candida glabrata

 

Risk factors for invasive candidiasis from multivariate analysis:

(1) total parenteral nutrition (TPN, OR 4.7)

(2) Candida colonization (OR 8.0; to be of use there should be a preoperative surveillance protocol)

 

Prior antibiotic use was a risk factor in the univariate analysis.

 

A patient considered to be at increased risk may benefit from closer surveillance and prophylactic antifungal therapy.
