Description

Teng et al reported risk factors for mortality in a patient treated for bleeding gastric varices. These can help to identify a patient who may require more aggressive management. The authors are from Chang Gang Memorial Hospital in Taiwan.


Patient selection: status post endoscopic treatment of bleeding gastric varices in a patient with cirrhosis

 

Predictors of 6-week mortality:

(1) impaired renal function (OR 21.1)

(2) high Child-Turcotte-Pugh score (OR 2.5), with score >= 9

(3) MELD score (OR 1.18 per point), with MELD >= 18

(4) rebleeding within 5 days (OR 16.4)

(5) acute on chronic liver failure (OR 4.7)

 

Predictors of rebleeding within 5 days after therapy:

(1) SIRS (OR 9.4)

(2) active bleeding seen at initial endoscopy (OR 6.7)
