Description

Someya et al identified factors contributing to exercise-induced oxygen desaturation for a patient with systemic sclerosis and interstitial lung disease. The authors are from Kanazawa University in Japan.


Patient selection: systemic sclerosis and interstitial lung disease with SpO2 at rest > 95%

 

Outcome: exercise-induced oxygen desaturation (>= 4% decline in oxygen desaturation during a 6-minute walk).

 

The key predictor was a reduced percent of predicted DLco. A cutoff of 56.3% was 0.83 sensitive and 0.86 specific with area under the ROC curve of 0.92.

 

Other findings in desaturators:

(1) low FVC as percent of predicted

(2) low FEV1 as percent of predicted

(3) low total lung capacity as percent of predicted

 

Measuring pulse oximetry during cycling or treadmill exercise seems simple enough to detect patients.
