Description

Bruun et al identify factors associated with 90-day mortality associated with Streptococcal soft tissue infections. These can help to identify a patient who may benefit from more aggressive management. The authors are from multiple institutions in Denmark, Germany, Norway and Sweden.

Patient selection: soft tissue infection due to Group A Streptococcus (Streptococcus pyogenes) or Streptococcus dysgalactiae

Predictors of 90-day mortality (from Lasso regression):

(1) male sex (odds ratio 5.1)

(2) age of the patient (odds ratio 1.15 per year)

(3) septic shock (odds ratio 2.0)

(4) failure to treat with intravenous immunoglobulin (odds ratio 3.2)

The risk of mortality increases with the number of risk factors present.
