Putnam et al reported a score the diagnosis of a necrotizing soft tissue infection in pediatric patients. This is based on a subset of the LRINEC score. The authors are from Children's Memorial Hermann Hospital of the University of Texas Houston, Seattle Children's Hospital and the University of Washington.
Patient selection: pediatric patient < 18 years of age with suspected necrotizing soft tissue infection
Parameter:
(1) serum CRP in mg/L
(2) serum sodium in mmol/L

Points
serum CRP
<= 20 mg/L
0
> 20 mg/dL
1
serum sodium
< 135 mmol/L
1
>= 135 mmol/L
0
score =
= SUM(points for the 2 parameters)
Interpretation:
• minimum score: 0
• maximum score: 2
• A score of 2 is associated with a high risk for a necrotizing soft tissue infection.
• The CRP is more sensitive, and sodium is more specific.
Performance:
• The area under the ROC curve is 0.84.
