Description

Injection of a contrast medium may be followed by the onset of a immune hemolytic anemia.


Clinical and laboratory features:

(1) recent history of an infusion of a contrast medium

(2) onset of hemolysis with sudden drop in hemoglobin after the administration of the contrast media

(3) evidence of an immune mechanism (positive antiglobulin test)

(4) exclusion of all other possible explanations (which can be challenging)

 

In theory re-exposure to the same contrast agent should trigger another hemolytic episode.
