Description

Wrong site surgery may involve a number of errors related to who, where and how.


Wrong site surgery may encompass:

(1) surgery done on the wrong patient

(2) surgery done on the wrong side (opposite to intended site)

(3) surgery done on the right side but wrong anatomic site (finger, eyelid, tooth, etc)

(4) surgery done on the right side and right site but wrong part

(5) wrong procedure performed

 

More than one of these errors may occur in a patient.

 

Wrong site surgery needs to be distinguished from a change in diagnosis or change in plan that is made after the operation has started.
