Description

Baclofen is administered intrathecally to treat otherwise intractable spasticity. Sudden discontinuation of baclofen can result in a potentially serious withdrawal reaction.


 

Baclofen is a GABA B receptor agonist.

 

Discontinuation of therapy can occur:

(1) intentionally

(2) accidentally, often by malfunction of the infusion pump used to deliver the drug

 

Features of the withdrawal syndrome:

(1) psychosis with hallucinations

(2) delirium

(3) fever

(4) spasticity

(5) seizures

(6) tachycardia

(7) autonomic dysfunction

 

Restarting baclofen therapy will cause the signs and symptoms to revert. Some patients may respond to oral therapy but some patients require restoration of intrathecal delivery.

 

Deaths have been reported but fortunately are rare.
