Description

Hoff et al used a weight-based extended-interval dosing protocol for gentamicin in critically-ill neonates. A simple protocol can often perform adequately. The authors are from Children's Hospital and Clinics of Minnesota in Minneapolis.


 

Patient selection: neonate receiving intensive care

 

Targets: peak plasma gentamicin concentration of 7-10 mg/L with trough concentrations less than 2 mg/L

 

Parameters:

(1) body weight at birth in grams

(2) concurrent therapy with indomethacin

 

Basic dose: 4 mg per kg given intravenously

Plasma trough and peak levels should be used to refine the drug regimen.
