Neuropathy Following Vinca Alkaloid Therapy Clinical Features

Purpose: To evaluate a patient for evidence of polyneuropathy following therapy with a vinca alkaloid.

Description

Following therapy with vinca alkaloids a patient may have axonal degeneration in both myelinated and unmyelinated nerve fibers, resulting in a reversible mixed sensori-motor-autonomic polyneuropathy.


 

Vinca alkaloids:

(1) vinblastine

(2) vincristine

(3) vindesine

(4) vinorelbine

 

Sensory findings:

(1) distal paresthesias in fingers and toes

(2) distal areflexia

(3) distal loss for light touch and pinprick

 

Motor findings:

(1) mild distal weakness

(2) facial weakness

(3) ptosis

(4) laryngeal paresis

(5) dysphagia

(6) vindesine may also produce proximal weakness

 

Autonomic findings:

(1) constipation

(2) urinary retention

(3) impotence

(4) orthostatic hypotension

(5) paralytic ileus
