Description

Traumatic injury to the urethra can be graded using the Urethra Injury Scale.


 

Type of Injury

Extent

Grade

hematoma or contusion

blood at urethral meatus with normal urethrography

I

stretch injury

elongation of urethra without extravasation on urethrography

II

disruption

partial, with urethrography showing both extravasation of contrast material and visualization of bladder

III

 

complete with < 2 cm urethral separation, without visualization of bladder on urethrography

IV

 

complete with >= 2 cm urethral separation

V

 

complete with extension into prostate or vagina

V

 

If more than 1 injury is present, then advance grade by 1 up to Grade III.

• I assume the advance in grade for multiple injuries applies for Grade II to III but not III to IV (see Testis Injury Scale).
