Description

Traumatic ulcerative eosinophilic granuloma with stromal eosinophilia (TUGSE) of the tongue is reactive ulcer of the tongue that is usually self-limited. Similar lesions may occur elsewhere in the oral cavity.


 

Synonym: granuloma eosinophilicum diutinum, traumatic granuloma, traumatized eosinophilic granuloma

 

It is the adult version of Riga-Fede Disease of the neonate or infant.

 

Clinical features:

(1) one or more ulcers on the tongue

(2) usually on the dorsal or lateral surface

(3) variable pain (from none to severe)

(4) variable history of trauma to the tongue

(5) often resolves after incisional biopsy or incomplete surgical removal

 

Histologic features:

(1) mucosal ulceration

(2) dense inflammatory infiltrate with lymphocytes, histiocytes and eosinophils

(3) variable presence of monoclonal T cells and/or CD30-positive lymphocytes

 

Differential diagnosis:

(1) cancer

(2) syphilis

(3) deep fungal infection

(4) hyperosinophilic syndrome
