Description

Mouridsen et al used an intravenous infusion of dexrazoxane (Savene) to treat patients with extravasation of doxorubicin or other anthracyclines. The authors are from multiple university hospitals in Europe.

The first infusion must be started within 6 hours of the extravasation.

There are 3 intravenous infusions:

(1) initially

(2) at 24 hours after the first

(3) at 48 hours

The dose is infused over 1-2 hours.

The dose is reduced by 50% if the creatinine clearance is < 40 mL per minute.

Dose

Creatinine Clearance >= 40 mL per minute

Creatinine Clearance < 40 mL per minute

initial (t = 0)

MIN(2000, 1000 mg * BSA in square meter)

MIN(1000, 500 mg * BSA in square meter)

24 hours

MIN(2000, 1000 mg * BSA in square meter)

MIN(1000, 500 mg * BSA in square meter)

48 hours

MIN(1000, 500 mg * BSA in square meter)

MIN(500, 250 mg * BSA in square meter)

Complications may include:

(1) mild pain

(2) mild sensory disturbances

(3) nausea

(4) local pain at the dexrazoxane injection site

(5) transient elevation of the ALT
