Description

The Multidimensional Health Assessment Questionnaire (MDHAQ) is a derivative of the Health Assessment Questionnaire (HAQ). It can be used to monitor a patient with a rheumatologic disorder over time and to evaluate the response to interventions. It is an effective screening instrument to detect problems with sleep, stress, anxiety and depression. The authors are from Vanderbilt University and the University of Kansas.

NOTE: The questionnaire is copyrighted by the Health Report Services.

Measures:

(1) abilities for activities of daily living and feelings, 4-point Likert scale

(2) visual analogue scale (VAS) for pain

(3) personal beliefs about the illness, 5-point Likert scale

(4) visual analogue scale (VAS) for fatigue

Abilities for activities of daily living and feelings

(1) consists of 18 items

(2) based on status at the present moment

(3) scored on a Likert scale from 1 (no difficulty) to 4 (unable to do)

VAS for Pain: Responses over a 7.7 cm line ranging from "no pain" to "pain as bad as it could be"

Personal beliefs:

(1) consists of 8 items

(2) scored on a Likert scale from 1 (strongly disagree) to 5 (strongly agree)

VAS for Fatigue: Responses over a 7.7 cm line ranging from "fatigue is no problem" to "fatigue is a major problem"

score for abilities =

= SUM(points for all 18 ability statements)

score for beliefs =

= SUM(points for all 8 belief statements)

Interpretation:

• minimum abilities score: 18

• maximum abilities score: 72

• minimum beliefs score: 8

• maximum beliefs score: 40

• The higher the scores, the more difficulty the patient is having with the condition.

Performance:

• The score does not show the floor effect (presence of normal scores despite reporting functional impairment) seen with the HAQ and MHAQ.

• The items show good test-retest reliability and face validity.
