Description

Hahn et al developed a 10-item instrument for evaluating the physician's experience of a recent patient encounter. This can help identify patients who are perceived as "difficult" or problematic for a variety of reasons. The authors are from multiple hospitals with the lead author from the Albert Einstein College of Medicine in New York City.


 

The instrument consists of 10 questions.

 

Forward directed questions: 2, 3, 4, 5, 6, 7, 9

Reverse directed questions: 1, 8, 10

= SUM(points for all 10 questions)

 

total score as percent of maximal frustration =

= ((total score) – 10) / 50 * 100% = ((total score) – 10) * 2%

 

Interpretation:

• minimum score: 10

• maximum score: 60

• The higher the score, the greater the difficulty for the physician.

• Patients who are viewed as difficult have in general: more mental disorders, greater functional impairment, higher health care utilization, lower satisfaction, and higher health care costs.
