Gass modified a staging classification used to describe formation of a macular hole. The author is from the Bascom Palmer Eye Institute in Miami, Florida.


 

Features

Stage

impending hole (yellow spot); early serous detachment of the foveolar retina

1A

impending or occult hole (yellow ring or halo); with a small ring there is serous detachment of the foveolar retina; with a larger ring there may be an occult foveolar hole

1B

small hole or tear in the contracted prefoveolar vitreous bridging the round retinal hole, with no loss of the foveolar retina

larger macular hole with or without a pseudo-operculum; posterior vitreous not detached

full thickness macular hole with posterior vitreous detachment from the optic disc and macula, with or without a pseudo-operculum
