Description

Olmesartan therapy can cause in a sprue-like enteropathy.


 

Olmesartan medoxomil is an angiotensin II receptor blocker used to treated hypertension.

 

Features of olmesartan-related enteropathy:

(1) signs and symptoms of celiac sprue (weight loss, diarrhea, anemia) starting after olmesartan therapy started

(2) villous atrophy on small bowel biopsy

(3) serologic tests for celiac sprue negative

(4) refractory to gluten-free diet

(5) usually responsive to corticosteroid therapy

(6) reversal of enteropathy after discontinuation of olmesartan therapy

• Since olmesartan is absorbed in the small intestine, it would be interesting to see if the patient antibodies to olmesartan or an olmesartan-protein complex.
