Description

A sperm granuloma is a focus of granulomatous inflammation that develops around a release of semen into adjacent tissues, including the dermis.

Features of a sperm granuloma:

(1) The patient has a history of surgery (usually vasectomy) or trauma to the vas, rete testis or epididymis.

(2) The affected area is usually asymptomatic but may be painful.

(3) Histology shows granulomatous inflammation. Sperm may be present but usually are absent.

(4) Other causes of granulomatous inflammation (tuberculosis, deep fungal infection, Schistosomiasis, sarcoidosis, sexually-transmitted disease, etc) are excluded.
