Description

Hanifin proposed a standardized grading system for patients with atopic dermatitis being treated with topical agents. Patients are assessed for response by grading the treated and untreated areas before and after treatment.


Parameters used to evaluate atopic dermatitis:

(1) erythema (requisite)

(2) induration or papulation (requisite)

(3) pruritis and excoriation (requisite)

(4) lichenification (for long term assessment)

(5) scaling and dryness (for long term assessment)

(6) erosion, oozing or weeping (for pediatric patients; generally not relevant to adults)

 

Parameters used for score to grade response to topical agents:

(1) erythema

(2) induration

(3) pruritis

 

Selection of comparison areas:

(1) pairs of comparable areas are selected, one to be treated with the topical agent and the other to be left untreated or treated with a placebo

(2) sites should not include the hands, face or thickly lichenified lesions

(3) sites should generally be 3-4 cm in diameter but can be as small as 2 cm

(4) areas are kept outlined with a marking pen during the study period

= (points for erythema) + (points for induration) + (points for pruritis)

 

Interpretation:

• minimum score: 0

• maximum score: 9

• The effectiveness of an agent can be evaluated by the change in score after application, relative to change in an untreated/placebo control area.

• For studies lasting 2 or more weeks, a global assessment of the patient should be made.
