Gossop et al described the Severity of Drug Dependence (SDS) questionnaire for interviewing patients with substance abuse problems. An earlier paper by Sutherland (which had Gossop as a co-author) called the same questionnaire the Opiate Subjective Dependence Questionnaire (OSDQ).
If a person abuses more then one drug, then the questionnaire should be repeated for each substance.
The instrument consists of questions about recent drug use.
(1) use of the substance out of control
(2) anxiety over missing a dose
(3) worry about substance use
(4) desire to stop
(5) expected difficulty to stop

Interpretation:
• minimum score: 0
• maximum score: 15
• The higher the score the greater the perceived dependence for the substance.
• A low score may indicate a person who is unaware of how bad his/her dependence actually is.
