Louis et al used a simple questionnaire to screen patients for essential tremor. This can identify those patients for whom a more formal evaluation is indicated. The authors are from Columbia University in New York City.

There could be a variety of ways to gather responses:
(1) yes or no, graded 1 or 0
(2) Likert scale, grading severity or frequency of tremor
Looking at the questions, a single positive response might be sufficient to justify a further investigation.
