Description

Thin-section CT images of the lung from a patient with idiopathic pulmonary fibrosis can be graded based using a scoring system. The score can be monitored over time and may correlate with the patient's prognosis.

Parameters:

(1) alveolar score

(2) interstitial score

Parameter

Finding

Points

alveolar score

no alveolar disease, normal

0

ground glass opacity involving < 5% of the lobe (minimal disease, but not normal)

1

ground glass opacity involving 5 - 24% of the lobe

2

ground glass opacity involving 25 - 49% of the lobe

3

ground glass opacity involving 50 - 75% of the lobe

4

ground glass opacity involving > 75% of the lobe

5

interstitial score

no interstitial disease

0

interlobular septal thickening

no discrete honeycombing

1

honeycombing and/or septal thickening involving < 25% of the lung

2

honeycombing and/or septal thickening involving 25 - 49% of the lung

3

honeycombing and/or septal thickening involving 50 to 75% of the lung

4

honeycombing and/or septal thickening involving > 75% of the lung

5

total score =

= (points for alveolar score) + (points for interstitial score)

Interpretation:

• minimum total score: 0

• maximum total score: 10

• The higher the score, the more severe the disease.
