Description

Diaz-Plasencia and Rebaza-Iparraguirre developed a score for grading the severity of colonic gangrene consequent to volvulus. It can help identify patients at high risk for mortality who may benefit from more aggressive therapy. In Spanish this was called the Puntaje de Severidad de Gangrena por volvulo de Colon (PSGC); in English the Score of the Severity of Gangrenous Colon by volvulus (SSGC). The authors are from Hospital Belen de Trujillo, Peru.

Parameters used for score:

(1) mean arterial blood pressure

(2) gross appearance of peritoneal fluid

(3) presence of a macroscopic colonic perforation

where:

• Items were weighted so that minor findings were scored 2 and severe were scored 0.

• mean arterial pressure = ((systolic blood pressure) + (2 * (diastolic blood pressure))) / 3

SSGC =

= SUM(points for the 3 findings)

Interpretation:

• minimum score: 0

• maximum score: 6

• The higher the score, the better the prognosis.
