Williams and Oz identified preoperative risk factors associated with mortality following placement of a mechanical left ventricular assistance device. This can help in obtaining informed consent from patients and family. The authors are from Columbia-Presbyterian Medical Center.
Parameters prior to placement:
(1) mechanical ventilation
(2) postcardiotomy shock
(3) pre-left ventricular assist device
(4) prothrombin time
(5) central venous pressure

= SUM(points for all 5 parameters)
Interpretation:
• minimum score: 0
• maximum score: 10
• The higher the score the greater the postoperative mortality.
• A score > 5 was associated with a 67% mortality rate.
Limitations:
• The score has not been validated in a prospective study.
• The reference range for prothrombin time needs to be given.
