Description

Vuorisalo et al identified risk factors for surgical site infection (SSI) in patients undergoing coronary artery bypass graft (CABG) surgery. These can help to identify patients who may benefit from more aggressive management to avoid infection. The authors are from the University of Oulu and University of Oulu Hospital in Finland.

Patient selection: CABG without valve procedure

Sites for surgical sites infection include:

(1) superficial incision for the donor site for blood vessels

(2) superficial incision for the chest area

(3) deep (organ space) for the chest area (mediastinum)

Risk factors for surgical site infection (SSI) include:

(1) body mass index (BMI) at an extreme (< 20 or >30 kg per square meter)

(2) female gender

(3) COPD

(4) diabetes (for SSI of chest area)

Avoidance of SSI involves:

(1) scrupulous skin preparation using an effective protocol

(2) use of effective prophylactic antibiotics

(3) special attention during emergency surgery
