Description

Rene et al identified risk factors for death in critically ill recipients of an allogeneic hematopoietic stem cell transplant. These can help identify patients who may require more aggressive management. The authors are from multiple hospitals in France.


 

Independent risk factors for mortality for all recipients:

(1) mechanical ventilation

(2) elevated bilirubin

(3) corticosteroid therapy for active graft-vs-host disease (GVHD)

 

where:

• Hepatic failure was defined as a total serum bilirubin >= 68 µmol/L (>= 4 mg/dL).

• Mechanical ventilation would indicated respiratory failure.

 

In patients who required mechanical ventilation, the risk of death increased along with the number of organ failures (shock, hepatic failure, etc.).

 

The 1 year survival for a patient who required mechanical ventilation was 11%.
