Description

Olsen et al identified a number of risk factors associated with surgical site infection (SSI) following a major breast operation. These can help identify a patient who may benefit from closer monitoring following the surgery. The authors are from Washington University and Barnes-Jewish Hospital in St. Louis.


 

Risk factors for a surgical site infection:

(1) insertion of a tissue expander or breast implant

(2) suboptimum antibiotic prophylaxis prior to surgery

(3) blood transfusion

(4) history of chest irradiation

(5) performance of a mastectomy

(6) recent or current smoking

 

Factors associated with a reduced risk of a surgical site infection:

(1) infiltration with a local anesthetic agent

 

where:

• The odds ratios for the risk factors range from 2.1 to 5.3.

• The infiltration with a local anesthetic agent may indicate a less extensive procedure.

• Giving optimum prophylactic antibiotics can be viewed as protective.
