Description

Murphy et al identified risk factors for prolonged ileus following colectomy. These can help to identify patients who may benefit from more aggressive management. The authors are from the University of Wisconsin in Madison.


 

Patient selection: adult underging colectomy

 

Outcome: prolonged postoperative ileus

 

Risk factors with higher odds ratios:

(1) diagnosis Crohn's disease or volvulus (versus cancer)

(2) ascites

(3) sepsis

(4) operative time >= 221 minutes

(5) age >= 74 years

(6) open approach

 

Risk factors with lower odds ratios:

(1) no bowel preparation

(2) no oral antibiotics

(3) age 53 to 73 years

(4) overweight or obese by BMI

(5) operative time 118 to 220 minutes

(6) chemotherapy

(7) smoking

(8) male gender

 

Factors associated with less risk of postoperative ileus:

(1) Asian race (versus White)

 

where:

• In the abstract weight loss is given as a risk factor but this is stated not to be a risk factor in Tables 3 and 4, page 283.

 

Patients with postoperative ileus are at increased risk with readmission, complications, longer hospital stays and reoperation.
