Description

Lautenbach et al identified risk factors associated with fluoroquinolone resistance in nosocomial infections with Escherichia coli and Klebsiella pneumoniae. These can help identify a patient who should be monitored more closely for failure to respond to therapy. The authors are from the University of Pennsylvania in Philadelphia.


 

Patient selection: nosocomial infection with E.coli or Klebsiella pneumoniae

 

Risk factors for fluoroquinolone resistance on multivariate analysis:

(1) recent fluoroquinolone therapy

(2) residence in a long-term care facility (nursing home, etc.)

(3) recent aminoglycoside therapy

 

Age was also listed as an independent risk factor, but adjusted odds ratio was only 1.03, compared to 3.7 to 8.9 for the other factors.
