Description

Jonard et al identified risk factors for acute renal failure (ARF) in a woman following delivery. These can help to identify a woman who may require more aggressive management. The authors are from Hopital Roger Salengro in Lille, France.

Patient selection: woman admitted to ICU postpartum

Outcome: acute renal failure

Risk factors for acute renal failure:

(1) HELLP syndrome (hemolysis, elevated LFT, low platelet count) AND postpartum hemorrhage

(2) infusion of hyperoncotic albumin during management of the hemorrhage

Significance:

(1) Any therapy that may worsen renal function should be avoided if possible..

(2) A woman with the HELLP syndrome should be monitored for hemorrhage, and vice versa.
