extensive (XDR)
all of the antibiotics listed above except polymyxins and tigecycline
carbapenem (CR)
imipenem or meropenem
Risk factors for extensive drug resistance:
(1) COPD
(2) bedridden
(3) central venous catheter
Risk factors for mortality:
(1) extensive drug resistance
(2) in the ICU
(3) concurrent fungal infection
(4) age >= 70 years
