Description

Cheng et al identified risk factors for pancreatitis following ERCP. These can help identify patients who may be at risk prior to the procedure or who may require closer monitoring afterwards. The authors are from Indiana University and multiple university medical centers in the Midwest.

Risk factors identified on multivariate analysis:

(1) minor papilla sphincterotomy

(2) sphincter of Oddi dysfunction suspected

(3) history of a previous episode of ERCP-related pancreatitis

(4) age < 60 years of age

(5) >= 2 contrast injections into the pancreatic duct

(6) a trainee involved in the procedure

where:

• The odds ratio were 3.8 for sphincterotomy, 2.6 for suspected sphincter of Oddi dysfunction, and 1.5 to 2.0 for the remainder of factors.

In an attempt to develop a risk score, I assigned points as follows:

Risk Factor

Points

sphincterotomy

2

sphincter of Oddi dysfunction

1.5

history of previous episode

1

age < 60 years

1

contrast injections

1

trainee involvement

1

total risk score =

= SUM(points for all 6 risk factors)
