Description

The sinusoidal obstruction syndrome (SOS) may develop in a patient following cytoreductive therapy or bone marrrow transplant, especially if certain risk factors are present.


 

Synonym: veno-occlusive disease of the liver

 

Risk factors for the sinusoidal obstruction syndrome - conditioning therapy:

(1) total body irradiation

(2) gemtuzumab ozogamacin (anti-CD33, used in AML)

(3) chemotherapy with cyclophosphamide, busulfan, carmustine, carboplatin, thiotepa and/or melphalan

 

Risk factors for the sinusoidal obstruction syndrome - patient factors:

(1) advanced age (arbitrarily used > 65 years in the implementation)

(2) pre-existing hepatic disease

(3) cytomegalovirus infection

(4) therapy with acyclovir, amphotericin B or vancomycin during conditioning (direct effect of antibiotic vs consequence of systemic infection)

 

Risk factors for the sinusoidal obstruction syndrome - transplant factors:

(1) donor HLA mismatch

(2) donor HLA identical but unrelated

(3) graft vs host disease
