Description

A patient with paraneoplastic hypercortisolism is at significant risk for severe infections.


 

Patient selection: tumor associated with paraneoplastic hypercortisolism

 

Risk factors for severe infection:

(1) hypercortisolism

(2) myelosuppressive therapy (chemotherapy and/or radiation)

 

The hypercortisolism can be reversed quickly by administering 2-4 grams of metyrapone in divided doses.

 

Other risk factors:

(3) bronchial obstruction (if lung cancer)

(4) immunodeficiency

(5) diabetes mellitus
