Description

Respiratory complications following coronary artery bypass graft (CABG) surgery can be predicted by certain preoperative risk factors. This can help identify patients at greatest risk for respiratory problems and to adjust perioperative management.


 

Respiratory complications after cardiac surgery:

(1) need for prolonged ventilation following surgery

(2) respiratory failure requiring reintubation

(3) pneumonia or tracheitis

(4) adult respiratory distress syndrome

(5) tracheostomy

(6) respiratory arrest

 

Factors associated with respiratory complications by univariate analysis and logistic regression:

(1) congestive heart failure at the time of surgery

(2) emergency procedure

(3) reoperation

(4) preoperative serum creatinine >= 1.9 mg/dL

(5) history of COPD with bronchodilator therapy

(6) aortic stenosis

(7) prior vascular surgery

(8) operation for mitral valve insufficiency

(9) age >= 70 years of age

Additional factors associated with respiratory complications identified by univariate analysis:

(1) severe left ventricular dysfunction

(2) mitral valve stenosis

(3) aortic insufficiency

(4) weight <= 65 kilograms

(5) preoperative hematocrit <= 34%

(6) history of COPD without bronchodilator therapy

(7) prior transient ischemic attack (TIA) or stroke

(8) female gender

(9) left main coronary artery disease

(10) history of congestive heart failure
