Risk factors for a healthcare-associated infection:
(1) being hospitalized for >= 2 days in the past 3 months (last 90 days)
(2) resident of a nursing home or long-term care facility
(3) receiving home infusion therapy
(4) home wound care
(5) requiring long-term dialysis in the past month (last 30 days)
(6) family member with a multi-drug resistant pathogenic micro-organism
