Certain chemotherapeutic agents can cause renal salt wasting (RSW) which can lead to hyponatremia (renal salt wasting syndrome or RSWS).
Chemotherapeutic agents implicated:
(1) cisplatin
(2) carboplatin
Mechanism: damage to renal tubules with impaired sodium resorption
Clinical and laboratory features:
(1) euvolemia
(2) hyponatremia
(3) elevated urine sodium concentration
(4) urine sodium excretion exceeds daily intake
Management often involves sodium supplementation.
