Description

The Red Scrotum Syndrome (RSS) tends to be a chronic disease in adult males with some uncertainty as to its pathophysiology.


 

Clinical features:

(1) intense erythema of the anterior scrotum in an adult male

(2) persistent pruritis

(3) burning sensation

(4) hyperalgesia

 

Key negatives:

(1) absence of scaling and excoriations

(2) extrascrotal sites not involved

 

A biopsy may show telangiectasias with no or little inflammation.

 

Some cases are associated with overuse of topical steroids. These cases will improve with steroid cessation.

 

Diagnosis requires exclusion of other conditions such as infection, allergy, psoriasis and ichthyosis.

 

Oral doxycycline is associated with improvement over time. Some use gabapentin as second-line therapy.
