Description

Rosenberg et al identified a number of red flags for a patient with lower urinary tract symptoms (LUTS). These can help to identify a patient who may benefit from more aggressive management including referral to a specialist. The authors are from Mid-Michigan Health Centers, Brown University and the University of Toronto.

Patient selection: LUTS

Red flags for referral:

(1) history of recurrent urinary tract infections (UTI)

(2) macroscopic or microscopic hematuria

(3) prior genitourinary surgery or trauma

(4) suspicion of neurologic cause for the symptoms

(5) suspicion of urinary retention

(6) urethral or meatal stenosis

(7) uncertainty in diagnosis

Additional red flags for a male:

(8) abnormal prostate exam

(9) rising PSA

An additional red flag might be history of tumor in the genitourinary tract or pelvis.
