The effectiveness of rehabilitation with a limb prosthesis following a lower limb amputation can be assessed using a simple questionnaire.
The instrument consists of 6 questions related to mobility.
The first 3 have responses scored from 0 (limited) to 3 (normal).
The last 3 are answered yes (0 points) or no (1 point).

= (points for waking hour use) + (points for limb usage) + (points for need of assistance device) + (points for use outside)
Interpretation:
• minimum score: 0
• maximum score: 12
• The higher the score, the better the rehabilitation.
