Three domains (physical function, adaptation and fears) are scored, with additional information collected to further characterize the patient. Interpretation:
• A score of 100 for a section indicates good health and function and implies a higher quality of life.
• A score of 0 for a section indicates very poor health or function and implies a lower quality of life.
