Description

Gladman et al identified risk factors which can be used to identify a patient with psoriatic arthritis at diagnosis who is at risk for disease progression. These patients may benefit from early initiation of aggressive therapy. The authors are from the Universities of Toronto and Waterloo in Canada.

Timing: first visit to the psoriatic arthritis clinic (irrespective of previous disease duration)

Risk factors associated with disease progression:

(1) high number (>= 5) of joints with effusions

(2) past history of therapy with steroids or a disease-modifying agent (gold, chloroquine, methotrexate, azathioprine, etc)

Risk factors associated with low risk of disease progression:

(1) an erythrocyte sedimentation rate (ESR) < 15 mm in the first hour
