Parameters:
(1) duration of infertility in years
(2) motility index (Macleod, 1969) = (percent motile sperm from 0 to 100) * (motility grade)
(3) sperm density in millions per mL
(4) age of man in years
where:
• percent motile sperm is normally >= 50%
• motility grade: none = 1, poor = 2, fair = 3, good = 4.
• oligozoospermia is when sperm density is < 20 million per mL; severe oligozoospermia is when the sperm density is <= 10 million per mL
