Description

Bustamante et al developed a prognostic index for evaluating a patient with hepatic encephalopathy associated with cirrhosis. This can help predict if a patient will have a fair or poor survival. The authors are from the University of Barcelona in Spain.

Parameters:

(1) gender (1 if male, 2 if female)

(2) serum bilirubin in mg/dL

(3) serum alkaline phosphatase, presumably in U/L

(4) serum albumin in g/L

(5) serum potassium in mmol/L

(6) serum BUN in mg/dL

(7) prothrombin activity in percent (from 0 to 100)

where:

• The methods for determining the serum analytes are not specified.

• The reference ranges for the serum analytes are not specified.

• The units for alkaline phosphatase are not specified, but the multiplication factor from the equation below would favor U/L rather than µKat/L.

prognostic index (PI) =

= EXP(X)

X =

= (-0.8782 * (points for gender)) + (0.1449 * (serum bilirubin)) + (0.0021 * (serum alkaline phosphatase)) - (0.0746 * (serum albumin)) + (0.3437 * (serum potassium)) + (0.0299 * (BUN)) - (0.0193 * (prothrombin activity))

Interpretation:

• A low PI was <= 0.4175.

• A high PI was > 0.4175.

• A cirrhotic patient and hepatic encephalopathy has a poor prognosis and should be evaluated for liver transplant.
