Description

Woo et al evaluated patients with primary cutaneous anaplastic large cell lymphoma. They identified patients who may benefit from more aggressive management. The authors are from Stanford University and Henry Ford Hospital in Detroit.


 

Patient selection: CD30 positive anaplastic large cell lymphoma

 

Exclusions: extracutaneous disease at presentation, transformation from mycosis fungoides, lymphomatoid papulosis and borderline lesions

 

Criteria for extensive limb disease - at least one of the following:

(1) single limb with disease encompassing a circular area > 15 cm in diameter

(2) multiple lesions involving 3 or more continguous body regions, including at least 1 limb

 

Risk factors associated with all-cause mortality:

(1) older age at diagnosis (>= 54 years of age)

(2) development of extracutaneous disease

 

Risk factor for disease-specific mortality and worse clinical outcomes (and candidates for aggressive management):

(1) extensive limb disease (ELD)

(2) development of extracutaneous disease
