Description

Hse et al evaluated risk factors for mortality in critically ill patients with systemic lupus erythematosus (SLE). This can help identify patients who may benefit from more aggressive therapy in the intensive care unit. The authors are from the National Taiwan University Hospital in Taipei, Taiwan.


 

In the study population, mortality was almost 50% with 80% of patients requiring corticosteroid therapy and almost 70% requiring mechanical ventilation. A quarter of the patients were admitted for pneumonia with ARDS.

 

Risk factors associated with mortality on multivariate analysis:

(1) septic shock (odds ratio 77)

(2) intracranial hemorrhage (odds ratio 19)

(3) gastrointestinal bleeding (odds ratio 7)

 

The APACHE II score (using a cutoff of > 19) was not significantly associated with death in the multivariate model.
