Description

Bosch et al identified prognostic factors for a patient with a mantle cell lymphoma. These can help identify a patient who may benefit from more aggressive management. The authors are from the University of Barcelona and multiple hospitals across Spain.


 

Factors associated with a lower response rate to therapy:

(1) Ann Arbor Stage IV

(2) high-intermediate or high risk International Prognostic Index (IPI)

(3) elevated LDH level

 

Factors associated with poor survival:

(1) ECOG performance scale >= 2

(2) splenomegaly

(3) B-symptoms (fever, night sweats, weight loss)

(4) mitotic index > 2.5 mitoses per high powered field (hpf)

(5) leukocyte count > 10,000 per µL (associated with leukemic MCL)

(6) elevated LDH level (above the upper limit of normal)

(7) blastic cytology

(8) high-intermediate or high risk International Prognostic Index

• The IPI includes age, ECOG status, Stage, serum LDH and extranodal involvement.
