Description

Kumar et al developed an Early Mortality Risk Score for evaluating patients with primary systemic amyloidosis. A patient at increased risk for early mortality may benefit from more aggressive management. The authors are from the Mayo Clinic.

Patient selection: primary systemic amyloidosis

Outcome: mortality within 1 year (early mortality)

Parameters:

(1) serum troponin in µg/L (Roche Diagnostics)

(2) serum N-terminal pro-BNP (brain natriuretic peptide) in ng/L (Roche Diagnostics)

(3) serum uric acid in mg/dL (Roche Diagnostics)

where:

• A serum uric acid had a reference range of 4.3 to 8.0 mg/dL for men and postmenopausal women and 2.3-6.0 mg/dL for premenopausal women.

• The reference range for serum troponin was not given.

• The reference range for N-terminal pro-BNP was not given.

• The testing was referred to as baseline and not from time of clinical onset or diagnosis.

total score =

= SUM(points for all 3 parameters)

Interpretation:

• minimum score: 0

• maximum score: 3

• The higher the score the greater the risk of 1 year mortality.
